------------- PHYSICAL EXAM FINDINGS ----------------
Attending Veterinarian: Dr. Abigail Duvall
Observational examination
Mentation: BAR
Posture: WNL
Overall appearance: WNL, overall apparently healthy, some damaged feathers
Vital stats
Heart rate: 300
Respiratory rate: 28
Auscultation: Clear heart & respiratory sounds; no murmurs or arrhythmias
Mucous membranes: Pink & moist
Hydration: Euhydration
Oral cavity: Clean & clear of debris; mild drainage of fluorescein stain from eyes
Choana: Clean & clear of debris
Glottis: Clean & clear of debris
Beak: Normal length & occlusion; no areas of cracking/fracturing
Cere/nares: Clean & clear of debris; no discharge
Eyes: Corneal scar OD, retinal scar OS, negative fluorescein stain uptake OU, no blepharospasms or discharge
Ears: Clean & clear of debris
Crop: No abnormalities observed on palpation
Feather quality: Several broken (but impable) feathers on the right wing (P1, P2, P3, P4, P5, P6, P8, P9, P10, S9, & S10) & the left wing (P9, P10, S1, S3, S4, & S5); all tail feathers are broken, but impable
Integument: Small, focal areas of featherless skin on left & right carpi (appears healed, with no areas of redness or scabbing); no other areas of abnormalities
Wings: No palpable fractures/abnormalities
Wings - Range of motion: Appropriate flexion & extension of all joints
Wings - Patagium: Intact with no areas of contracture bilaterally; no areas of swelling or cause for concern bilaterally
Legs/Feet: No palpable fractures/abnormalities
Legs/Feet - Range of motion: Appropriate flexion & extension of all joints
Legs/Feet - Plantar surface: Small eschar present on the right hallux; several small eschars on digits 1, 2, 3, & 4 of the left foot
Legs/Feet- Talons: Appropriate length & sharpness
Neurologic: Appropriate mentation & awareness of surroundings; no observed cranial nerve deficits; no proprioceptive deficits
Ceolom: No abnormalities observed on palpation
BCS: 3--low muscle tone but large amount of sc fat
Vent: WNL; healthy, pink tissue; no soiling of feathers surrounding vent
Droppings: Not observed
Uropygial gland: Swelling of right side of uropygial gland (palpation reveals likely cystic in nature); static from presentation; aspirate revealed yellow-tinted, clear, thin fluid with no odor (~0.5 mL removed); no evidence of active infection; no abnormalities associated with the left side. Stain (diff quick) only oily material, acellular.
Problem list:
1. Multiple broken, but impable feathers on the left & right wing, as well as the tail
2. Corneal scar OD
3. Retinal scar OS
4. Over-conditioned
5. Cystic uropygial gland (R side only)
6. Small eschars on plantar surface of left & right digits
Assessment: Patient appears systemically healthy & will be ready to be released once broken feathers are imped as long as this leads to return of flight ability (right now patient fails flight assessment, though no clinical reason for this is found on physical examination--history of coracoid/clavicle fractures but palpate as well healed).

**This bird is one of a group from the same area that may be good to try and release in one trip -- 230 from Laverne, 237 and 24 from Enterprise and 268 from Troy

Plan: Tx: feather imping
Collected blood for CBC/chem
Prognosis: Good if imping resolves flight issues.

During catch up for weighing patient escaped and was able to fly and gain lift in treatment room. Patient flew several times. When patient caught up, noted white opacity at approximately 9 o clock near limbus of right eye. Fluorescein stain of both eyes revealed corneal ulcers OU at limbus and small scratches on cornea in the right eye.
Ketorolac discontinued, neopolygram OU BID started. Patient is already set to have recheck w/ophtho at some point.

Comments
Anesthestized with Iso/O2 via mask. HR/RR monitored throughout, maintained at 3%, no complications.

VD, H-view, and Lat images of thoracic girdle obtained. Images show some callus formation at site of coracoid fracture - the small bone fragments caudal to the distal fracture segment appear to have formed callus to each other and toward the fracture site. There is a very faint amount of bridging visible between the distal segment and the proximal segment still attached to the keel. There remains significant caudal displacement at the fracture site. The clavicles appear irregular with a possible fracture line visible just distal to midline on the lateral view. Alignment is adequate.

On physical exam we noted stringy saliva in the mouth, and the ulnar vein appeared to have poor profusion. We also noted poor jugular fill and that the patient was approxitley 7% dehydrated. We ran a PCV and total solids which came back a PCV of 39% and TS of 6.4. We also estimated a 1% buffy coat.We decided to give a 50 mL fluid bolus in the right stifle due to the severe hypovolemia we appreciated on physical exam.

VD View: Significant asymmetry and lucnecy (proximal) were observed along the right coracoid, just cranial to the keel.
The findings were notable for abnormal gas patterns, suggesting a possible injury or infection. There is a possible moderately displaced fracture of the distal left scapula that is not visible on the H view.

H-View: A fracture/luxation of the right coracoid was observed. The fracture was transverse, possibly along the junction of the proximal coracoid and keel and ventrally displaced. The asymmetry and gas observed on the VD view corresponded to the findings seen on the H-View, further confirming the presence of a significant injury to the right coracoid.

Lateral View: Fracture of the coracoids were noted bilaterally, with associated small fracture fragments. Both fractures are located just proximal to the keel and are displaced both caudally and dorsally when compared to the keel's position. The right coracoid fracture was consistent with the findings on the VD and H-View. Lucencies noted within the intestines as well as decrease opacity of the GI tract overall.
